The Hawthorns Surgery offers a professional and caring service to our patients, utilising the skills and enthusiasm of our team members.
We feel that the service we give to our patients is of paramount importance therefore you are registered with a specific Doctor (your named GP) not the Practice as we aim to provide efficient, accessible, friendly and high quality family health care. We strive to work together as a practice team and in partnership with our patients, in order to promote health and develop mutually beneficial relationships.
The Hawthorns Surgery is also a recognised Training practice and is instrumental in developing and nurturing the next generation of doctors, from medical students through to qualified doctors undertaking GP specialist training. The understanding and support of our patients in this area is invaluable and we believe the practice and patients as a whole benefits from this important work.
Dear Patients, The Hawthorns would like to assure you all, we are still very much here and available for you during these difficult times. We are offering telephone, video consultations with our GP's and Nurses on a daily basis Monday to Friday. We are also offering a face to face GP appointment if deemed necessary after clinical judgement is made by the GP over the phone. Please if asked to attend a face to face appointment with a GP or Nurse, wear a face covering/mask. During this Covid-9 Pandemic we as a practice have had to quickly adapt our ways of working to ensure safety for you and our team. Due to this you may be offered an appointment at our local network surgery 'Ashfield' in Walmley, please be assured however you will be seen by one of our own Hawthorns Surgery clinician. Eligible patients will now receive a text message from the surgery inviting them to book their Covid vaccination at Sutton Town Hall. If your mobile number has changed please let us know. If you are registered for Online Services please log on and check that we have the correct contact details for you.
Patients without a mobile will be called up in the usual manner.
Our local vaccination centre has now moved to a new venue. All future vaccinations will take place here. Follow up appointments you may have had at Falcon Lodge and Ley Hills have now all moved to the new location at Sutton Town Hall. The times remain the same as before.

LATEST UPDATE ON GPDPR Extraction (July 2021)
The current NHS Digital (NHSD) extract of GP data for Research purposes (known as the GPDPR) has been delayed due to NHSD wishing to review the way in which this data will be collected, to conduct more public involvement and information about the plans and change the way in which patients can opt out of the extract of their GP data.
Currently the only way to opt out is to complete a Type 1 opt out form and return this to the practice by the 1st September. However, this extract will not be taken until the NHSD have changed the way it will take the data and respect the patient’s choice for using their data. NHSD are introducing the following changes to the opt out process which will mean that patients will be able to change their opt-out status at any time::-
- Patients do not need to register a Type 1 opt-out by 1st September to ensure their GP data will not be uploaded.
- NHS Digital will create the technical means to allow GP data that has previously been uploaded to the system via the GPDPR collection to be deleted when someone registers a Type 1 opt-out.
- The plan to retire Type 1 opt-outs will be deferred for at least 12 months while we get the new arrangements up and running and will not be implemented without consultation with the RCGP, the BMA and the National Data Guardian.
This means that you can opt out at any time in the future and NHSD will delete data that they already have taken for research purposes, the deadline of the 01/09/2020 has been delayed until a new system of opt out is developed. Hopefully, this will be a simple centralised approach via the NHS app or NHS website to avoid paper form and administration work for your GP.
We will update you when we know more about the NHSD plans to change how you can control who has access to your data.

Change of Second Vaccination Date
You may have heard recently that ALL patients are being advised to have the second vaccination sooner.
Over the next few weeks, all patients affected by this will be contacted by the practice and offered a new appointment date.
Please do not contact the practice yourself to enquire about moving your appointment. We have a significant number of patients we need to call and appreciate you patience in this matter.
Covid Vaccination Guidance
Following recent reports, we know that many people may be concerned about the risk of blood clots after having their Covid-19 vaccination.

Proof of your vaccination status will be available on the NHSapp, (which is also valuable for accessing your health records and ordering repeat prescriptions)

Alternatively, you can call the NHS helpline on 119 (from 17 May) and ask for a letter to be posted to you. This must be at least 5 days after you’ve completed your course of the vaccine, the letter may to take up to 5 days to reach you.

IMPORTANT CHANGES TO TELEPHONE ACCESS FROM 09th of APRIL 2021
In order to improve telephone access the following times are being designated for accessing different services at the surgery. Calls for appointments will be accepted throughout the Surgery opening hours. (Please note: appointments can only be offered subject to availability)
Prescription queries, will only be dealt with between 11AM TO 3PM.
Medical Secretary queries, will only be dealt with between the hours of 10AM to 11AM and 3PM to 4PM.
Results will only be dealt with after 3PM.

Designed to be used with Google Chrome, Firefox, Safari, and Internet Explorer 9 and above browsers.
Patients can register for online services, with our provider company and booking / cancelling appointments (currently suspended) and repeat prescription requests currently available. To register for these services there is a registration form available on the right hand side of this screen, please complete and return to Reception. Please note Photo ID will need to be provided.
If you are 16 or over and wish to also have either summary access to your medical records or detailed access online which is: please complete the additional form and bring with your Online Registration Form.

Registering at the Practice is easy with online forms available to download and appointment slots to suit most patients’ timetables. You’ll enjoy a confidential service in comfortable surroundings from a helpful and dedicated team.
